No treatment is likely to reverse the changes of lichen sclerosus completely, but the symptoms and signs of the disease can usually be well controlled with the application of a steroid. If the symptoms return, you should go back to using the cream daily for 2 weeks before reducing the usage again when your symptoms eases. Studies in animals have shown that corticosteroids applied to the skin of pregnant animals can cause abnormalities in the development of the foetus, such as cleft palate or retarded growth of the foetus in the womb.

If you’re worried about the side effects, read the patient information leaflet for further information. Prolonged use of topical corticosteroids (particularly on the face) should be avoided. Please see MHRA Drug Safety Update September 2021 for further information on the risk of topical steroid withdrawal reactions.

Intravenous stress dose hydrocortisone and diflucan were given, serum total calcium normalized and oral thrush improved. Despite that clobetasol cream had been stopped, he was hypertensive (blood pressure140/90), and angiotensin-converting enzyme inhibitor (ACEI) was started. • The active ingredient in Eumovate is clobetasone, a moderately potent corticosteroid that reduces redness, itching and swelling.
  • Sometimes you may be prescribed them under the supervision of a skincare specialist (dermatologist).

If you are on regular medication you should discuss these effects with your doctor/health care team before becoming pregnant. As part of routine antenatal care, most women will be offered a very detailed scan at around 20 weeks of pregnancy to check the baby’s development. Women who have used a systemic corticosteroid in early pregnancy will not usually need any extra monitoring.

Topical Corticosteroid: MODERATE Potency

Very potent topical corticosteroids are not usually prescribed for pregnant or breastfeeding women, or for very young children. Sometimes you may be prescribed them under the supervision of a skincare specialist (dermatologist). The active ingredient in Betnovate is betamethasone valerate, which is a potent corticosteroid. Because it’s a stronger steroid, Betnovate is usually used to control outbreaks rather than as a long-term preventative measure.

Advice for healthcare professionals:

Creams are better for moist, weeping areas of skin, while ointments are most suitable for drier, scaly areas. The skin on the face is thinner and absorbs steroid more easily, making it more vulnerable to damage by the steroid. Be aware that babies’ nappies will act as an occlusive dressing and so should not be used if you have applied a steroid to the baby’s bottom. The skin may also bruise more easily and become more susceptible to infection.

In cases where your eczema is infected you may be prescribed a topical corticosteroid that also contains an antibiotic or antiseptic. They are not a cure as they have no effect on the underlying cause of the inflammation, but they do control the flare-up and relieve the symptoms, such as itching and redness. If a new medicine is suggested for you during pregnancy, please ensure the doctor or health care professional treating you is aware of your pregnancy.

For more information on side effects, see the leaflet that comes with the medicine. Sometimes, the amount of medicine you’re advised to use will be given in fingertip units (FTUs).

However, given the number of patients who use topical corticosteroids, we understand reports of severe withdrawal reactions to be very infrequent. Side effects are most common in the elderly and very young children — or if you use topical steroid creams for long periods. These topical steroids can also spread the infection further from the affected areas of the body. However, rare side effects can include withdrawal, glaucoma, and cataracts.

One fingertip unit (approximately 500 mg from a tube with a standard 5 mm diameter nozzle) is sufficient to cover an area that is twice that of the flat adult handprint (palm and fingers). Potency of a topical corticosteroid preparation is a result of the formulation as well as the corticosteroid. The potency of steroid should be matched to the severity of the disease, using the least potent steroid that effectively controls the disease. Most adults and children can use topical corticosteroids safely, but there are situations when they are not recommended.
